Online Check-In is a state-of-the-art, online appointment scheduling system.
How Online Check-in works?
Online Check-in allows you as the patient to take care of the "check-in" registration forms prior to arriving for your visit, You can then schedule your own appointment at a time that is convenient for you. This allows for shorter wait times and less paperwork when you arrive at one of our centers.
Is my information secure?
Yes. Your personal information is encrypted and is then securely delivered electronically to our physical office.
How long does it take?
Less then 5 minutes if you have your insurance information available. Less than 3 minutes if you are Self Pay.

Please note: Upon completing the online check in, your information will be submitted to the clinic. A member of our staff will call you shortly with an appointment time.
1) Please print and complete the attached forms and bring them with you to the clinic.
2) You will also need to bring a photo ID and your insurance card.
If you have not been contacted within 30 minutes (during normal business hours), please call the clinic to verify receipt of your file. Thank you for using our online check-in system.
